What Areas of the State does ICG Home Health Care Services cover?
Our services can be provided in Chicago and the Chicago Metropolitan area. We recommended that your doctor/hospital issue a referral to us directly in order for services to begin in your home.


Is HMO insurance accepted?
HMO insurance is accepted upon referral from your Primary Care Physician. Please contact your Primary Care Physician to issue you a referral for the services you are seeking and inform them you are requesting that the services are provided by ICG Home Health Care Services.

If ICG Home Health Care Services is not a member of your HMO medical group your physician can recommend an agency within your HMO medical group that can provide the sought after services.


How do I know if my insurance will provide coverage for the therapy services that we need?
On the back side of your insurance card you will find a phone number for the customer service line of your insurance provider. You can call this number to verify whether your insurance will cover the requested therapy service. Otherwise, if you are unsure if your insurance company covers the requested therapy services, please contact Beata Hryszko at beatah@icgrehab.com or (312) 943-3600 Ext. 333. She will gladly verify benefits with your insurance company to determine if you are able to receive services from ICG.


What if I need to cancel or reschedule my services?
If you need to cancel/reschedule a service should occur, please contact the individual that is to provide that service FIRST. The individual providing you the service should have provided you with a contact number/business card. If this did not happen or if you have lost or misplace that information, contact Beata at our office (312) 943-3600 Ext. 333 as soon as possible. She will then consult with the service provider to reschedule your cancelled visit.
